Take Note of Changes to the Secretary of State’s Medical Report Form

The Illinois Secretary of State's Medical Report form has been modified to address the most frequently asked questions by physicians. In addition, the name of the form has been changed from "Medical Report" to "Medical Report for Conditions that May Impair Driving Safely."
- Key changes to the form include:
- Modifications to Section II-Medical Health, Question #2(a) to inform physicians that the Secretary of State does not monitor hypertension or hyperlipidemia; if the patient’s only medical condition is hypertension or hyperlipidemia, do not check "Yes" next to "Cardiovascular." The Secretary of State’s office noted that many drivers are unnecessarily monitored because "Yes" is checked incorrectly.
- Mental health will no longer be separated from physical health on the form.
- The condition "Developmental" has been added to the form.
- Questions regarding patients who have medication-related side effects that would impair safe driving and whether the driver is compliant with medication regimes have been added.
- A question as to how long a healthcare professional has been treating a particular driver has been added.
