Cardiovascular Disease Risk Reduction Initiative Accepting Applications through September 4

The Centers for Medicare & Medicaid Services (CMS) is accepting applications from physicians and other health care professionals to enroll in the Million Hearts® CVD Risk Reduction initiative.  Health care professionals who participate will work with Medicare beneficiaries to determine their risk for a heart attack or stroke within the next 10 years and identify the best approaches to reduce their risk.  

Physicians will be provided payment for reducing the absolute risk for heart disease or stroke among their high-risk Medicare patients. Patients will receive a personalized risk modification plan that will target their specific risk factors.

The model aims to enroll 300,000 Medicare beneficiaries through 720 practices across the United States. Interested physicians must submit a letter of intent and an official application. Letters of intent may be submitted beginning July 6, 2015; applications must be received by Sept. 4, 2015.
