Did You Receive a Gray Post Card from IDFPR?

If you have not completed or verified the accuracy of your online profile with the Illinois Department of Financial and Professional Regulation (IDFPR), you will be the recipient of a gray post card.

The post card will reinforce that your medical license will expire July 31, 2014, and that IDFPR will not renew your medical license if you have not completed your online profile, which is required by Illinois law.

As you may recall, physician online profiles were reinstituted in Illinois during August 2011 as part of the Patient Right to Know Act

How to complete, verify your profile

First, access your profile online. Enter your social security number (or PIN number included on the post card), and your Illinois physician license number. Next, complete or verify the accuracy of your online profile.
