Attention Medical Entities: Paper Applications for Corporate Licenses Soon to Be Replaced

If you need to obtain a corporate license for a medical entity, you'll have to jump online to fill out the application.  

Effective Sept. 1, 2017, the Illinois Department of Financial and Professional Regulation (IDFPR) will no longer accept paper applications for obtaining (or renewing) a license for a Medical Corporation, Limited Liability Company or Professional Service Corporation.

IDFPR is migrating professions and entities to its Online Services Portal

Medical entities are required by law to register with IDFPR to obtain a license annually. If you already have a corporate license, don't worry. Regardless of the date on which the license was issued, corporate licenses expire at the end of each year.

Please note: Medical Corporations, Limited Liability Companies or Professional Service Corporations must be licensed separately from a physician’s individual medical license and controlled substance license(s).
