  • Avoid a Scary Lapse in Medical Licensure: Renew by Halloween

    If you did not renew your medical license and/or controlled substance license by the July 31, 2014, deadline, there's still time left.

    But don't delay. Your medical license will lapse if your renewal isn't completed by Oct. 31, 2014.

    And prepare for the sticker shock: The renewal fee is $690 plus a $230 late fee, totaling $920.

    To renew your medical license, paper submission is now the only option:

    • Print out and complete this form from the Illinois Department of Financial and Professional Regulation's website (online renewal is no longer available).
    • Include a check for $920 with your renewal form.
    • Submit proof of 150 hours of Continuing Medical Education (CME).
    • Mail all materials to:
      Department of Financial and Professional Regulation
      320 W. Washington Street
      PO Box 7086
      Springfield, IL 62791
    • Postmark by Oct. 31, 2014.

    The controlled substance licenses are still $15 per location and may be renewed with the medical license.

    So before trick-or-treaters begin ringing your door bell, be sure your license renewal paperwork is in the mail!
