  • Medical Cannabis Final Rules Published; Here is How to Certify Qualified Patients

stylized bottleThe final regs to steer the Compassionate Use of Medical Cannabis Pilot Program are a done deal, and the Illinois Department of Public Health has posted the necessary forms to get the certification and application processes started.

What do physicians need to know?

To certify qualified patients for medical cannabis, physicians must submit this completed form to the Illinois Department of Public Health (cannot be sent in prior to September 2).

Note: Physicians and their practices must send in the certification. The form should not be given to patients and/or caregivers.

NEW: Medical Cannabis, an updated medical legal guide for ISMS members only, gives details on certification, lists applicable medical conditions, and provides guidance on prohibitions that apply to the Compassionate Use of Medical Cannabis Pilot Program Act.

How do patients and caregivers apply?

The patient application process for registering identification cards is divided into two submission periods:

    • Patients with a last name beginning with A through L may submit applications in September and October.
    • Patients with a last name beginning with M through Z may submit applications in November and December.

These applications also cannot be sent in prior to September 2. However, beginning Jan. 1, 2015, applications will be accepted year round.

Here are the online applications for qualifying patients:

    Patient Application Form

    Caregiver Application Form

    Fingerprint Consent Form
