Personal Health Record Pamphlets
The Illinois State Medical Society is here to help you educate Illinoisans on how to be smarter patients. One of the easiest ways patients can become better health care partners with you is to bring vital medical information to each doctor appointment. This new personal health record pamphlet is part of ISMS' "Help Us Help You" patient education campaign.
ISMS members may order a maximum of 50 copies by sending an e-mail to Lori O'Connor at oconnor@isms.org. Please specify quantity and write "Help Us Help You" in the e-mail subject line.
